Decades of Progress at a Crossroads: How Cuts and Inaction Threaten the Fight against HIV

An ISGlobal policy paper

13.11.2025

The number of HIV cases has fallen in sub-Saharan Africa, but has risen in at least 28 countries in other regions, particularly in Eastern Europe, Central Asia, the Middle East, and North Africa.

Faced with this reality, the cuts announced by the Global Fund's main donors—the United States and several European countries—threaten to reverse years of progress and put millions of lives at risk. A new analysis by ISGlobal warns that the cuts applied by the United States alone could lead to up to 3 million additional HIV-related deaths and more than 10 million new infections by 2030.

“Now that we know so much about the virus, how to treat it and prevent it, we cannot go backwards,” says the document, which urges European and Spanish leaders to strengthen their leadership in global health governance. ISGlobal proposes a series of recommendations to prevent the definitive erosion of global health initiatives and strengthen the health autonomy of the Global South.
